Single User Request (Student or staff) #
Jisc Online Surveys (Formerly BOS or Bristol online) are easy to create and maintain, and the results can be exported into Excel or PDF.
Please raise a job via Support Me using the ‘Request an Online Surveys account’ service, if you would like a Jisc online Surveys account – this is available to staff and students.

Multiple Student Request #
To request an Online Survey account for multiple students, please download the Excel file Multiple Student Request – Online Survey and Save it to an area on your H Drive. Then open the file and complete the fields required. The saved file can be added to the ‘Request an Online Surveys account’ request on Support Me.
